What an editor decides
Editors are described by the journal as scientific stewards responsible for rigor, integrity, and patient relevance, applying consistent standards across study designs, clinical settings, and geographic contexts.
Initial screening covers scope alignment, the quality of the title and abstract, and whether the work adds knowledge beyond an incremental or redundant analysis. A manuscript is rejected when it is out of scope, methodologically flawed, or lacks clinical relevance.

How review is organized
Manuscripts that proceed to external peer review are normally evaluated by at least two independent subject-matter experts.
Revisions are assessed by comparing the authors' responses against the reviewers' comments. Appeals are handled with a documented rationale and, where needed, further independent review.

Ethics and competing interests
Editors require conflict-of-interest disclosure, funding statements and author contribution details, and screen for plagiarism, duplicate publication and image manipulation.
